2. Certificate of Incorporation or similar document
3. Operating Agreements, Partnership Agreement, Board Meeting Minutes, or other documentation (i.e. Power of Attorney, Certificate of Authority) defining the person(s) authorized to sign bank documents including a Resolution to open a bank account

Ownership Identification (UBO) - shareholders with 25% or more ownership
Copy of the company shareholder register or equivalent document
Owner/Director Identification - a valid & legible clear copy of Identification (Passport, DL, State ID Card, etc.).
Proof of Principal Owner/Director Current Address - a valid & legible copy of driver’s license, recent bank statement, utility bill, recent mortgage statement from a recognized lender, tax documentation.

 If any principal owner is another company, provide Company Documents described above:
. Merchant Processing Statements - Most recent 6 months statements (if not start-up)
. Bank Statement - Most recent 3 months statements (if not start-up)
. Financials - copies of most recent annual and current interim Financial Statements
. Marketing Materials - Supporting documentation describing the nature of the business activity. Recommendations include; 
1) Brief Business Overview on company letterhead; company description, description of goods/services, management structure, ownership, company history, etc., 
2) Detailed business plan, especially for start-up companies and/or 
3) Copies of sales and marketing collateral, company presentations or electronic copies of relevant web pages.
. Refund Policy - a copy of refund policies/return procedures (recommended)
. URL’s - a Listing of all URL’s, with test username and password
. URL ownership – copies via whois; or invoice/screenshot from Registrar backend
